Minimally invasive (MICS) or even endoscopic cardiac surgery (ECS) has become an increasingly adopted alternative to conventional sternotomy, offering advantages such as reduced trauma, shorter hospital stays, and faster recovery. However, the limited access inherent to MICS or ECS presents significant technical challenges, particularly in the domains of aortic occlusion and myocardial protection. The choice between endoaortic balloon occlusion and transthoracic cross-clamping remains controversial, with both techniques demonstrating distinct advantages and limitations depending on the patient’s anatomy, surgical approach, and institutional experience. Similarly, myocardial protection strategies—ranging from antegrade and retrograde cardioplegia to beating-heart techniques—must be tailored to ensure optimal cardiac preservation under the constraints of minimally invasive access. This webinar aims in showing the current techniques and evolving strategies surrounding aortic occlusion and myocardial protection in MICS and ECS, highlighting the importance of individualized approaches and the need for further research. Standardized protocols could help to improve safety and outcomes. Advances in imaging, perfusion technology, and surgical tools continue to refine these methods, aiming for the ideal balance between efficacy, reproducibility, and minimal invasiveness.
