An ischemic time period of 4 hours is used as the acceptable threshold for clinical heart transplantation in many centers. The preservation solutions used for storage of donor hearts during this period vary by heart transplant centers both nationally and internationally. The limitations of mechanical arrest and hypothermic preservation combined with other clinical challenges can contribute to primary graft dysfunction (PGD), which accounts for the majority of deaths after heart transplant. Preservation solutions can be broadly categorized as intracellular and extracellular solutions. The Universities of Giessen and Sao Paolo heart transplant programs have used histidine-tryptophan-ketoglutarate [HTK] solution exclusively for donor heart preservation since decades, which represents a real-world experience with this intracellular preservation solution protection. Our experts will discuss both adult and pediatric cardiac preservation.
