The emergence of organ transplantation as an accepted therapeutic procedure is primarily due to the refinement of surgical techniques and the development of immunosuppressive drugs, together with the introduction of better methods for organ preservation and protection. Today, the continuously rising demand for donor organs can no longer be met from the numbers of potential organ donors. This means that new strategies will have to be devised to provide additional donor organs, while steps must be taken to ensure that all available donor organs are fully utilized. Organ preservation is of crucial importance because it provides precious time for all the complex interdisciplinary preparations. Prolongation of cold ischemia time gives an opportunity to select a recipient who has optimal compatibility and allows the transplant to be transported over longer distances. The objectives and strategies of both surgical preparation and preservation of the organs are reviewed in this webinar. The physiological and pathological events occurring during visceral preparation and preservation will be considered, the consequent requirements regarding techniques and solutions will be outlined and lastly their clinical relevance to each organ will be discussed.
